Position Summary
The Infusion Pharmacist is responsible for overseeing the compounding, dispensing, and clinical management of sterile and non-sterile infusion medications. This role ensures compliance with state and federal regulations, USP <797>/<800> standards, and accrediting body requirements (ACHC/URAC). The pharmacist will serve as a clinical resource for nurses, physicians, and patients to ensure optimal outcomes in home infusion therapy.
Key Responsibilities
• Verify and compound sterile IV preparations, including antibiotics, TPN, IVIG, biologics, and hydration therapies.
• Oversee quality control, aseptic technique, and adherence to USP <797>/<800> guidelines.
• Conduct clinical assessments and medication reviews to optimize therapy outcomes.
• Provide patient education on infusion medication administration, side effects, and safety.
• Collaborate with prescribers and nursing staff to clarify orders and adjust therapy as needed.
• Maintain accurate records for dispensing, compounding logs, and controlled substance documentation.
• Ensure all pharmacy operations meet ACHC, Board of Pharmacy, and HIPAA requirements.
• Participate in accreditation audits and continuous quality improvement activities.
• Provide clinical support for home infusion nurses and coordinate infusion scheduling.
Qualifications
• Required: Doctor of Pharmacy (PharmD) or BS in Pharmacy.
• Licensure: Active, unrestricted North Carolina pharmacist license.
• Experience:
• Minimum 2 years in infusion, sterile compounding, or hospital pharmacy preferred.
• Familiarity with USP <797>/<800>, ACHC/URAC standards, and home infusion processes.
• Strong communication and clinical documentation skills.
• Ability to work collaboratively in a team-based care model.
Preferred Experience
• Experience with TPN, IVIG, and complex biologic therapies.
• Prior exposure to EMR/infusion software (e.g., CPR , CareTend, Brightree).
• Leadership or supervisory experience in a sterile compounding setting.
