Position Summary

The Community Health Navigator plays a key support and coordination role on the KCC care team. This position focuses on:

  • Helping Community Health Workers (CHWs) and care teams find and maintain up-to-date resources
  • Building and strengthening partnerships with community programs and service providers
  • Supporting outreach and community events to increase KCC’s visibility and impact


The Community Health Navigator does some direct client-facing work, but is primarily focused on resource navigation, relationship-building with external partners, and helping the team connect clients to the right services quickly and efficiently.

Key Responsibilities

Resource Navigation & Support for CHWs

  • Research and maintain an organized directory of community resources (housing programs, food assistance, behavioral health, legal aid, employment services, financial assistance, etc.).
  • Respond to CHW and care team requests for resources and referrals, helping match clients with appropriate programs.
  • Monitor changes in eligibility, waitlists, and program requirements and keep the team updated.
  • Assist with problem-solving when clients face barriers accessing services (e.g., documentation issues, waitlists, insurance questions).


Partnership Development & Coordination

  • Reach out to community-based organizations, clinics, social service agencies, and other partners to learn about their programs and build referral relationships.
  • Participate in introductory calls, site visits, and meetings with external partners to explore collaboration.
  • Help develop and maintain simple workflows, referral processes, and points of contact between KCC and partner agencies.
  • Represent KCC in coalition meetings, provider networks, and workgroups as assigned.


Outreach & Community Presence

  • Support CHWs with outreach activities, including tabling at community events, health fairs, and resource events.
  • Assist in planning and preparing outreach materials (flyers, resource lists, contact sheets, etc.).
  • Help share information about KCC services and programs with community members and partners.


Documentation & Data Tracking

  • Document partner contacts, resource information, and referral pathways in KCC’s systems.
  • Track outreach activities, partnership meetings, and resource usage to support reporting and program improvement.
  • Provide feedback on resource gaps and community needs to leadership and care teams.

Qualifications

Required:

  • High school diploma or equivalent.
  • 1–2 years of experience in community health, social services, customer service, outreach, or a related role (or strong interest and lived experience working with the community).
  • Strong communication skills and comfort talking with both community members and professionals at other organizations.
  • Good organizational skills and attention to detail; able to track multiple resources, contacts, and referrals.
  • Ability to work independently and as part of a team in a fast-paced, community-based environment.
  • Basic computer skills (email, spreadsheets, data entry; experience with databases or case management systems is a plus).
  • Ability to travel locally using personal or public transportation.


Preferred:

  • Associate’s or Bachelor’s degree in public health, social work, human services, psychology, public administration, or a related field.
  • Experience working with low-income, uninsured, immigrant, or medically underserved communities.
  • Familiarity with Medi-Cal/Medicaid, Medicare, and/or local safety-net systems.
  • Bilingual or multilingual (e.g., Spanish, Cantonese, Mandarin, Tagalog, etc.).

Working Conditions

  • Combination of office-based work, virtual meetings, and community-based activities.
  • Occasional field work and attendance at community events, which may include evenings or weekends.
  • Ability to lift up to 25 lbs. (outreach materials, supplies) and stand or walk for periods of time during events or outreach.
