We are a busy outpatient neurology practice seeking an experienced Chronic Care Management Coordinator to support patients with chronic neurological conditions between office visits. This position involves regular patient outreach, care plan updates, medication and treatment coordination, documentation of CCM/PCM time, and communication with providers, pharmacies, insurance companies, and caregivers.
The ideal candidate has prior experience with CCM, PCM, care coordination, or case management and is comfortable managing a high volume of patient communication and documentation. Neurology experience is preferred but not required. Experience with eClinicalWorks, Chronic Care IQ, prior authorizations, specialty pharmacy workflows, or Medicare care management programs is a strong plus.
This role requires excellent organization, clear communication, compassion, follow-through, and the ability to recognize when a patient concern should be escalated to a provider.
Responsibilities include:
- Conducting monthly patient check-ins
- Documenting care coordination time
- Updating care plans
- Supporting patients with chronic neurological conditions
- Coordinating medications, refills, labs, authorizations, and specialty pharmacy needs
- Communicating with caregivers, pharmacies, insurance plans, and providers
- Escalating urgent or complex clinical concerns appropriately
- Maintaining accurate documentation in the EMR and CCM platform
Qualifications:
- Prior CCM, PCM, care coordination, case management, or medical office experience required
- Strong phone, documentation, and organizational skills required
- Neurology or specialty practice experience preferred
- Experience with eClinicalWorks or Chronic Care IQ preferred
