The Patient Care Coordinator is a vital member of a multidisciplinary healthcare team that includes a provider, Behavioral Health Consultant, Licensed Practical Nurse (LPN), Registered Medical Assistant (RMA), Care Coordinator, and Patient Service Representative (PSR). Together, the team works to provide high-quality, patient-centered care for an assigned panel of patients.
In this role, the Patient Care Coordinator provides planned care for patients identified by diagnosis or condition, those who are part of vulnerable populations, and patients transitioning from hospitals or other healthcare facilities. The coordinator helps identify and address barriers to care-such as transportation, financial concerns, or access to community resources-to support patient compliance with care plans and ensure essential appointments are completed. Additional responsibilities may include participating in population health programs and collaborating with the healthcare team to improve patient outcomes.
Minimum Qualifications:
- Demonstrate an understanding of customer service principles.
- Knowledge of available community resources and the ability to evaluate resources and make appropriate referrals (preferred).
- Knowledge of health-related financial assistance programs, including Medicare and Medicaid (preferred).
- Ability to interpret and comply with federal, state, and local laws and regulations.
