Key Responsibilities
- Serve as the primary point of contact for families navigating early intervention services
- Coordinate and monitor individualized family support plans
- Facilitate meetings, schedule services, and track progress
- Advocate for family needs with empathy and professionalism
- Maintain accurate documentation in compliance with program standards
- Collaborate with service providers, educators, and community partners
Qualifications
- Bachelor’s degree in social work, Early Childhood, Human Services, or related field (or equivalent experience)
- Experience in case management, early intervention, or family support services preferred
- Strong communication, organizational, and problem-solving skills
- Ability to work independently and as part of a team
