What are the responsibilities and job description for the Patient Care Specialist position at Katahdin Valley Health Center?
Note: The stated pay rate for each position reflects the budgeted amount for the position. Final offers will depend on your skills and experience relevant to the position.
The Patient Care Specialist serves as the first point of contact for patient requests that fall outside of scheduled visits but require timely, organized follow-through. This role manages the high volume of day-to-day patient needs — including medication refill requests, minor symptom triage routing, and medical paperwork that would otherwise fall to the clinical team. The goal of this role is to keep the care team focused on direct patient care by ensuring these requests are handled efficiently, accurately, and with great patient experience from the first interaction. The Patient Care Specialist will provide high-quality care support through telephone-based patient interactions and in person interactions when applicable, ensuring timely, accurate, and thorough documentation in the electronic medical record (EMR). As a key member of the Integrated Care Team, this role collaborates with providers, care teams, and administrative staff to deliver coordinated, patient-centered care in accordance to KVHC protocols and standards of care expectations. MA, LPN, or RN License required.
