Position Overview

The Human Services Case Manager (Qualified Intellectual Disabilities Professional – QIDP) is responsible for coordinating services for individuals in our day program and ensuring all supports, documentation, and programming align with each person’s goals and state requirements.

This role works closely with individuals served, direct support staff, families/guardians, and outside agencies to ensure services are consistent, compliant, and person-centered.

Key Responsibilities

  • Develop, implement, and monitor Individual Service Plans (ISPs)
  • Conduct assessments and track individual progress toward goals
  • Coordinate and monitor services within the day program
  • Maintain accurate documentation and ensure regulatory compliance
  • Communicate regularly with families, guardians, and interdisciplinary teams
  • Support best practices in service delivery and person-centered planning
  • Advocate for individuals served to promote independence and community participation

Qualifications

  • Bachelor’s degree in Human Services, Social Work, Psychology, Sociology, Special Education, or related field (required)
  • Ability to qualify as a QIDP under Illinois requirements
  • At least 1 year of experience working with individuals with intellectual or developmental disabilities (required)
  • Strong communication, organization, and documentation skills
  • Ability to work effectively as part of a team

Work Schedule

  • Monday – Friday
  • 8:00 AM – 4:00 PM
  • No nights or weekends
