Clinical & Human Services Professionals (RN, QIDP, LPN)

Current Openings:

Registered Nurse (RN) / Qualified Intellectual Disabilities Professional (QIDP/Case Manager)

Full-Time
Schedule: Monday-Friday | 8:00 AM-4:00 PM

This unique dual-role combines nursing leadership with person-centered case management. You'll oversee healthcare services while helping develop and coordinate individualized supports that improve quality of life for the individuals we serve.

Responsibilities include:

• Provide nursing oversight and clinical guidance
• Monitor health conditions and coordinate medical care
• Oversee medication administration and regulatory compliance
• Complete nursing assessments and maintain accurate documentation
• Develop, implement, and monitor Individual Service Plans (ISPs)
• Coordinate services with physicians, specialists, families, guardians, and community providers
• Participate in interdisciplinary team meetings
• Advocate for the health, safety, and personal goals of individuals served

Qualifications:

• Current Illinois Registered Nurse (RN) license required
• Bachelor's degree preferred
• Meets or is eligible to meet Illinois QIDP requirements preferred
• Experience working with individuals with intellectual or developmental disabilities is preferred but not required

Licensed Practical Nurse (LPN)

Part-Time or Full-Time

Schedule:

• Monday - Friday: 8:00AM - 4:00 PM or
• Monday-Thursday: 9:00 AM-3:00 PM, Friday: 9:00 AM-2:45 PM

Responsibilities include:

• Assist with medication administration and health monitoring
• Maintain medical documentation and health records
• Coordinate routine healthcare needs
• Collaborate with the RN, Direct Support Professionals, and program staff
• Support a safe, healthy, and person-centered environment

Qualifications:

• Current Illinois LPN license required
• Excellent communication and organizational skills
• Experience working with individuals with developmental disabilities is a plus
