Current Openings:
Registered Nurse (RN) / Qualified Intellectual Disabilities Professional (QIDP/Case Manager)
Full-Time
Schedule: Monday-Friday | 8:00 AM-4:00 PM
This unique dual-role combines nursing leadership with person-centered case management. You'll oversee healthcare services while helping develop and coordinate individualized supports that improve quality of life for the individuals we serve.
Responsibilities include:
- Provide nursing oversight and clinical guidance
- Monitor health conditions and coordinate medical care
- Oversee medication administration and regulatory compliance
- Complete nursing assessments and maintain accurate documentation
- Develop, implement, and monitor Individual Service Plans (ISPs)
- Coordinate services with physicians, specialists, families, guardians, and community providers
- Participate in interdisciplinary team meetings
- Advocate for the health, safety, and personal goals of individuals served
Qualifications:
- Current Illinois Registered Nurse (RN) license required
- Bachelor's degree preferred
- Meets or is eligible to meet Illinois QIDP requirements preferred
- Experience working with individuals with intellectual or developmental disabilities is preferred but not required
Licensed Practical Nurse (LPN)
Part-Time or Full-Time
Schedule:
- Monday - Friday: 8:00AM - 4:00 PM or
- Monday-Thursday: 9:00 AM-3:00 PM, Friday: 9:00 AM-2:45 PM
Responsibilities include:
- Assist with medication administration and health monitoring
- Maintain medical documentation and health records
- Coordinate routine healthcare needs
- Collaborate with the RN, Direct Support Professionals, and program staff
- Support a safe, healthy, and person-centered environment
Qualifications:
- Current Illinois LPN license required
- Excellent communication and organizational skills
- Experience working with individuals with developmental disabilities is a plus
