Job Overview

Responsibilities
- Conduct comprehensive patient assessments to determine health status and needs.
- Develop and implement individualized care plans based on patient assessments.
- Administer medications and treatments as prescribed by physicians.
- Monitor patients' vital signs and report any changes to the healthcare team.
- Provide education to patients and families regarding health management and disease prevention.
- Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
- Maintain accurate medical records and documentation in compliance with healthcare regulations.
- Adhere to infection control protocols to ensure patient safety.

Experience
Candidates should have a valid Registered Nurse license and relevant clinical experience. Preferred qualifications include:
- Experience in home community care settings or similar environments.

- Strong understanding of infection control measures within healthcare facilities.
- Previous experience working in Assisted Living Facility is advantageous.
