Job Title: Care Coordinator
Job Classification: Hourly, Non-Exempt
Reports To: Quality Improvment and Compliance Manager
Work Schedule: 40 hours; Monday through Friday
Primary Responsibilities:
Care Coordinators work within the Outpatient Department providing support to individuals and families entering or accessing Kanza’s behavioral health services, through supporting the intake/access to care process, coordinating referrals and follow-ups, and providing ongoing monitoring of care and needs following the treatment plan and providers. Care Coordinators help individuals and families new to Kanza successfully access and navigate Kanza’s programs and services, as well as external community resources and services to ensure whole-person integrated care according to the person-centered or family-centered treatment plan. Care coordinators manage ongoing coordination of care for a caseload of individuals or families to include facilitating ongoing collaboration among treatment providers and linkage to needed services, this would include phone calls and sending or receiving confidential patient information to ensure the providers on the treatment plan have necessary information for the integrated and seamless care of the individual or family. Care Coordinators work closely with a multidisciplinary team of professionals and the goal is to help improve client engagement in needed treatment services and help improve their quality of life outcomes.
Essential Job Duties including but not limited to:
1. Provide warm and friendly customer service in person, by phone, and in writing to individuals and families receiving services from Kanza, as well as to Kanza and external care providers to facilitate ongoing care coordination, collaboration, and communication.
2. Understand and follow HIPAA and 42 CFR Privacy Laws for patients with mental health and substance use disorders to ensure confidentiality and privacy.
3. Develop and maintain a positive working relationship with Kanza’s care providers, as well as community providers to ensure good coordination and care for patients.
4. Assist individuals and families entering Kanza’s services with the intake process and paperwork, including appropriate behavioral health and physical health screeners, patient consents, releases of information, and referrals.
5. Complete administrative work in a timely and accurate manner, most of which will occur in the patient electronic health record. This includes proper documentation of all contacts and activities pertaining to care coordination in the electronic health record system.
6. Link individuals and families with needed resources and services according to the treatment plan and regularly monitor and ensure that services are meeting their needs.
7. Completes comprehensive surveys with individuals at the appropriate frequency depending on the survey type.
8. Responsible for tracking and administering surveys as well as data entry.
9. Support individuals in completion of applications for applicable benefits to include KanCare Medicaid, non-emergency transportation resources, and other social services.
10. Utilize supervisory supports appropriately and participates in all team/program meetings.
11. Complete annual and assigned trainings on time and participate in ongoing professional development to enhance the care and services provided, including recovery-oriented, trauma-informed, person- and family-centered care.
12. Other duties as assigned.

- Intermediate computer skills and proficiency required to be able to quickly and accurately type, email, send electronic faxes, make/receive phone calls, and navigate and document in the electronic health record system.
Minimum Qualifications:
· Associate’s degree preferred but not required
· High School Diploma or equivalent accepted
· At least two years of applicable work experience that supports the skills and requirements of the position
Supervisory Responsibilities:
· This is not a supervisory position
Physical Demands:
· Frequently required to sit for extended periods of time;
· Regularly required to move around the building;
· Must be able to physically make and receive phone calls and speak to numerous individuals and providers frequently and extensively.
· Must be able to physically use a keyboard and mouse on a laptop or computer system.
Work Environment:
· Primary work location is the Hiawatha Main Office with occasional travel to outreach offices or community settings as needed

· May have exposure to verbally or physically escalated individuals
