Registered Nurse (RN) Case Manager

- Hours: Monday-Friday 8:00 a.m. -5:00 p.m.
- Holiday requirement, 1 major holiday and 1 minor holiday per year

Position Description:
We are seeking a compassionate Registered Nurse (RN) to join our interdisciplinary hospice team in providing high quality, expert care to our seriously ill patients. Our RN Case Manager is a vital part of our hospice team providing comfort, care, support and education to our patients and their loved ones.
The Ideal Candidate Will Possess the Following Requirements:
- Valid, unencumbered Missouri and Kansas RN or compact license to include both states
- Minimum of one year bedside experience in caring for hospice patients is preferred
- Ability to communicate effectively and professionally with others; and to prepare and present information in oral and written formats
- Superior organizational skills with the ability to multitask and meet deadlines; must schedule and manage time effectively
- Valid driver’s license and insurance plus reliable transportation
- CPR certification required
