Position Summary
Kamelya Home Health is seeking an experienced and compassionate Director of Patient Care Services (DPCS) to lead and oversee all clinical operations of our Medicare-certified home health agency. The DPCS is responsible for the coordination, supervision, and delivery of patient care services while ensuring compliance with California Title 22 regulations, Medicare Conditions of Participation, and agency policies and procedures.
The ideal candidate is a Registered Nurse with strong leadership skills, extensive home health experience, and a commitment to delivering high-quality patient-centered care.
Essential Duties and Responsibilities
- Direct and oversee all patient care services provided by the agency.
- Ensure compliance with federal, state, and local regulations governing home health services.
- Supervise and provide clinical oversight to nursing, therapy, medical social work, and home health aide staff.
- Review, implement, and monitor patient care plans to ensure quality outcomes.
- Coordinate patient admissions, recertifications, transfers, and discharges.
- Participate in Quality Assurance and Performance Improvement (QAPI) activities.
- Monitor patient outcomes and implement corrective actions when necessary.
- Conduct clinical record reviews and ensure documentation compliance.
- Provide leadership, mentoring, and performance evaluations for clinical staff.
- Develop and maintain clinical policies and procedures.
- Collaborate with physicians, referral sources, and interdisciplinary team members to coordinate patient care.
- Assist with survey preparation, accreditation reviews, and regulatory audits.
- Ensure adequate staffing and clinical coverage to meet patient care needs.
- Participate in agency leadership meetings and strategic planning initiatives.
- Be available during operating hours or designate a qualified alternate in accordance with California regulations.
- Serve as a clinical resource and provide direct patient care coverage when necessary to support agency operations and patient care needs. Responsibilities may include, but are not limited to, Start of Care (SOC), Resumption of Care (ROC), Recertification, Comprehensive Assessments, Evaluation visits, Discharge (DC) visits, supervisory visits, OASIS assessments, and other skilled nursing services as required. Ensure all clinical documentation is completed accurately and timely in the WellSky/Kinnser EMR system and in accordance with Medicare Conditions of Participation and California home health regulations.
Qualifications Required
- Current and unrestricted California Registered Nurse (RN) license.
- Meets California Title 22 DPCS requirements:
- Bachelor's degree or higher in Nursing or a health-related field and at least three (3) years of experience within the last five (5) years in a home health agency, primary care clinic, or healthcare facility, including one (1) year in a supervisory or administrative role; OR
- Four (4) years of experience within the last five (5) years in a home health agency, primary care clinic, or healthcare facility, including one (1) year in a supervisory or administrative role.
- Strong knowledge of Medicare Conditions of Participation (CoPs), California Title 22 regulations, OASIS-E requirements, home health documentation standards, and quality improvement processes.
- Must be highly proficient in the WellSky/Kinnser EMR system, including clinical review, OASIS management, scheduling oversight, physician orders, plan of care review, and compliance monitoring.
- Excellent leadership, organizational, communication, and problem-solving skills.
- Proficiency with Microsoft Office applications and electronic medical record systems.
Preferred
- Previous experience as a DPCS, Clinical Director, Director of Nursing, or Clinical Manager in a Medicare-certified home health agency.
- Experience with CHAP, ACHC, or Joint Commission surveys and accreditation standards.
- Bilingual skills preferred.
Physical Requirements
- Ability to travel to patient homes and agency locations as needed.
- Ability to sit, stand, walk, bend, and lift up to 25 pounds.
- Valid California driver's license, reliable transportation, and current automobile insurance.
