Our therapists provide rehabilitation in the environments where life actually happens—including homes, workplaces, schools, gyms, community settings, recreational venues, and other real-world environments.
Rather than asking, "How far can this client walk in the clinic?" we ask:
- Can they safely navigate a grocery store?
- Can they return to work or school?
- Can they hunt, fish, garden, attend sporting events, or travel?
- Can they safely access their home and community?
- Can they participate in meaningful family and social activities?
Treatment sessions are individualized, functional, and often highly creative.
Primary Patient Population
Our primary population includes individuals recovering from:
- Traumatic Brain Injury (TBI)
- Spinal Cord Injury (SCI)
- Polytrauma
- Amputation
- Neurologic disorders
- Vestibular and balance impairments
- Complex orthopedic injuries
- Catastrophic injuries related to motor vehicle and work-related accidents
Both acute and chronic cases are served.
Collaborative Team Approach
Most clients have a rehabilitation team that may include:
- Physical Therapists
- Occupational Therapists
- Speech-Language Pathologists
- Case Managers
- Physicians
- ATPs and DME providers
- Caregivers and family members
Our therapists work closely with all stakeholders to develop individualized, client-centered treatment plans focused on maximizing independence and quality of life.
Responsibilities
- Perform comprehensive evaluations and develop individualized plans of care
- Deliver evidence-based physical therapy interventions in home and community settings
- Address mobility, balance, strength, endurance, transfers, wheelchair mobility, community reintegration, adaptive equipment, and functional independence
- Collaborate with interdisciplinary team members and referral sources
- Provide patient, caregiver, and family education
- Participate in equipment evaluations and recommendations as appropriate
- Complete timely and accurate documentation

Qualifications
- Licensed Physical Therapist in Michigan (or eligible for licensure)
- Strong communication and problem-solving skills
- Ability to work independently in community settings
- Interest in neurologic rehabilitation, catastrophic injury, and community reintegration
- Experience with TBI, SCI, neurologic rehabilitation, wheelchair seating/mobility, or complex rehabilitation is preferred but not required
