Overview

We are seeking a dedicated Primary Care Physician Assistant or Family Nurse Practitioner to join our growing healthcare team in Saint Paul, Minnesota. This provider will help launch and grow a new primary care service line within our established addiction medicine and psychiatry clinic.

Our clinic has served the community for over 10 years and we have expanded into three integrated service lines: addiction medicine, psychiatry, and primary care. We specialize in a wraparound care model designed to support patients struggling with substance use disorders and mental health conditions, many of whom face barriers accessing traditional primary care.

This role is ideal for a self-starter who is passionate about serving underserved populations and interested in helping build a new clinical program. Once the patient panel grows, additional providers will be hired to support the service line.

Position Details

• Full-time position
• Monday–Friday schedule (8-hour shifts) 8 am–5 pm
• It is salary for one month then you are on production
• Expected patient panel: 16–20 patients per day once ramped
• In-person role, and will be required to travel one day a week to Otsego (40 min NW of the cities), one day a week to a local St Paul treatment site, and 3 days in clinic
• Dedicated care team to assist you

Responsibilities

• Conduct physical examinations and obtain patient medical histories
• Review intake blood work with patients in their follow up appointments
• Diagnose and treat common acute and chronic conditions in adult patients (18 )
• Order and interpret diagnostic tests (labs, EKG, X-ray)
• Develop and implement patient treatment plans
• Monitor patient progress and update electronic medical records
• Provide preventive health education and chronic disease management
• Collaborate with psychiatry and addiction medicine specialists for integrated care

Qualifications

• Licensed Physician Assistant (PA-C) or Nurse Practitioner (FNP)
• Minimum 2 years of experience in Family Medicine or Internal Medicine
• Active Minnesota license in good standing
• Ability to interpret EKGs and basic X-rays
• Experience with EMR systems preferred
• Strong knowledge of HIPAA regulations and patient confidentiality
• Ability to perform basic clinical procedures such as vital assessments, triage, and catheterization

Compensation and Benefits

• Salary: $130,000 per year (salary during ramp period, transitioning to production model with patient referrals) once on production salary can reach upwards of $160,000

Location Requirement

Applicants must be able to commute to or relocate to Saint Paul, MN (55114) prior to starting.
