Position: Travel Home Health Clinical Manager RN

Location: Honolulu, HI

Pay: $69 to $70/hour

Experience: 3 years of experience required. (Must have travel experience-first-time travelers will not be considered.)

Education: BLS and BSN Certifications are required.

Type: Full-time; Contract

Schedule: Day Shift, 8:00am to 4:30pm

Kahu Malama Nurses is seeking a Travel Home Health Clinical Manager RN to join a growing and dynamic team!

Job Description

Clinical Care & Assessments

• Complete Start of Care, Admissions, Recertifications, Resumption of Care, Transfers, and Routine Visits
• Perform thorough clinical assessments, including significant changes in patient condition
• Accurately document patient status using OASIS and clinical documentation standards
• Monitor and record vital signs and overall patient progress
  
  

Case Management & Coordination

• Coordinate patient care services and collaborate with interdisciplinary teams
• Develop and adjust care plans to meet patient goals
• Ensure compliance with infection control precautions and safety standards
  
  

Specialized Nursing Skills

• Wound care, including pressure ulcer prevention, staging, and management
• Wound VAC therapy management
• Medication and pain assessment/management, including use of appropriate pain scales
• Catheter care: urinary and suprapubic insertion and management
• Drain management
• IV therapy & central line/port care
• Phlebotomy services
