Come join us at Kahl Home! We are a Faith-Based, Nonprofit, Short-Term Rehabilitation and Skilled Geriatric Care facility sponsored by the Carmelite Sisters for the Aged and Infirm since 1955. Our state-of-the-art services, amenities, and excellent staff make Kahl Home a premier nursing facility in the Quad Cities.
We are seeking to hire a Social Worker.
Social Worker Qualifications:
- Must have at least 3 years of Long Term Care experience.
- IA Social Worker License or a Master's Degree in Social Work.
- Knowledge of MDS 3.0 and Point Click Care Knowledge of Electronic Medical Records (EMRs).
- Read, write, and speak English fluently.
Social Worker Job Summary:
- Assist with necessary documentation for the resident’s admission to the facility.
- Evaluate and address resident needs and concerns.
- Facilitate patient/family discharge to next level of care.
- Complete required documentation in PCC.
- Interview Resident and family to assess resident psycho-social strengths/ weaknesses.
- Update resident files/charts with the Discharge Plan, Progress Notes, Plan of Care and Assessment.
- Participate in the Interdisciplinary Care Conference Meetings to assess and review Residents’ progress.
- Assist resident and/or family members with issues concerning but not limited to emotional and financial needs.
- Assist in communication with families and notify them of new and apparent needs or any changes of the level of care.
- Conduct duties of the Admission Coordinator when coordinator is not available.
