This is a full-time on-site role for a MDS Coordinator located in Ewa Beach, HI. The MDS Coordinator will oversee and manage the Minimum Data Set (MDS) assessments and processes to ensure compliance with state and federal regulations. Responsibilities include developing and maintaining resident care plans, coordinating interdisciplinary care team meetings, evaluating residents' care needs, and providing support for geriatric nursing care. Collaboration with healthcare staff is required to ensure optimal care outcomes and accurate documentation.
- Licensure and certification as a Licensed Practical Nurse (LPN)
- Proficiency in MDS processes and assessments
- Skills in care planning and developing individualized care plans
- Experience and knowledge in geriatric nursing practices
- Strong attention to detail and compliance with regulations
- Excellent communication, organization, and teamwork skills
- Proficiency in healthcare software systems is a plus
- Experience in a long-term care or skilled nursing facility is highly preferred
