JOB SUMMARY DESCRIPTION:
The Medical/Educational Coordinator functions as a critical support resource within the emergency children's shelter, responsible for facilitating services and resources for children in crisis. This role focuses on case management duties that avoid clinical responsibilities, ensuring compliance with TC3 standards while promoting a safe and supportive environment for all residents.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Intake and Assessment:
- Conduct initial assessments to understand the needs of each child upon arrival.
- Gather relevant information and documentation required for case management purposes.
- Service Coordination:
- Schedule Medical, Dental, and Vision appointments for shelter clients.
- Enroll clients into school if they are of school age.
- Transport clients to and from school and appointments.
- Maintain School and Medical client records.
- Obtain appropriate documentations needed for School and Medical.
- Other duties as assigned.
- Collaborate with staff and external service providers to coordinate educational, recreational, and support services for children.
- Develop and maintain resource directories for community services, including housing, food, and educational support.
- Individualized Service Planning:
- Work with the Clinical Director to create and implement individualized service plans in collaboration with children and staff, aiming to address their specific needs and goals.
- Assist in monitoring progress and adapt service plans as necessary to ensure ongoing support and compliance.
- Facilitation of Supportive Interactions:
- Assist in organizing and facilitating structured groups and individual activities promoting life skills development, socialization, and emotional resilience without clinical involvement.
- Ensure children engage in safe and constructive interactions within the shelter.
- Documentation and Reporting:
- Maintain accurate and up-to-date records of case management activities, including service plans and interactions.
- Prepare reports for management and regulatory compliance, ensuring adherence to TC3 guidelines.
- Community Engagement:
- Foster positive relationships with local agencies, schools, and community organizations to enhance available support services.
- Represent the shelter during community meetings, advocating for the needs of children in emergency situations.
- Compliance and Training:
- Participate in ongoing training and development to stay current with best practices and TC3 compliance regulations.
- Adhere to all policies and procedures related to child safety and welfare.
Qualifications:
- Bachelor’s degree in social work, Human Services, or a related field (preferred), although, Experience in a related field may qualify.
- Experience in case management, particularly with children or youth in crisis situations.
- Strong communication, organizational, and interpersonal skills
- Ability to work collaboratively with diverse stakeholders including children, families, and community organizations.
