Registered Nurse - Interim Housing Program

Position Purpose: 

The Interim Housing Registered Nurse will participate as a care team member of the Enhanced Services Program, which provides intensive case management services, behavioral healthcare and offers a satellite medical clinic, on-site. This is funded by DHS, in the Housing for Health program. 

Principal Responsibilities:

1. Participates in all case conferences regarding ESP clients, supporting care team’s in treatment plans.
2. Conducts nursing assessments for new admissions. 
3. Completes TB screenings/readings and documentation, as needed. 
4. Administers oral, IM, SC, and SL medications as needed, during clinical hours. 
5. Assists the Medical Provider with procedures as necessary 
6. Complete corrective action plans, if any, post internal/external audits. 
7. Monitors medication management process to assist clients/patients in medication self-administration. 
8. Provides guidance and support to ESP staff, regarding medical care, as needed.
9. Takes patient complaints. Follows appropriate procedures in resolving any complaint.
10. Provides patient/client education and teaching. Schedules patient hospital or doctors (specialist) appointment, if necessary.
11. Assigns daily nursing duties. Identifies and corrects patient flow deficiencies.
12. Organizes and maintains medication inventory; to include removing expired medications and packaging them for proper disposal, in coordination with LVNs.
13. Preparation and dispensing of all medications as ordered by the Provider; to include the completion of dispensed medication labels.
14. Orders medical supplies as needed.
15. Collaborates with the Program Director and assists with the continuing improvement of systematic functions.
16. Develops and monitors the communication network between the multi-disciplinary ESP staff and external professionals to improve continuity of care and to avoid conflicting information conveyed to clients/patients regarding their treatment plan.
17. Ensure all PHI information is maintained in accordance with HIPAA regulations. 
18. Support program leadership in delivering all required services and meets outcome measures established by the Department of Health Services. 
19. Ensure quality measures, regulatory standards and customer service standards are met.
20. Ensure all PHI information is maintained in accordance with HIPAA regulations. 
21. As needed, assist Program Director with reports on operational issues, organizational opportunities and/or programmatic goals.
22. Monitors and maintains adherence to the Nursing Training Manual and Agency Policies and Procedures.
23.  Performs other duties as required by the program and/or clinic.

Requirements:

1. Licensed by the State of California to practice as a Registered Nurse.
2. CPR on an annual basis. Training may be provided.
3. At least one year experience in an ambulatory care setting.
4. Experience working with patients and staff from diverse socio-economic, ethnic, and cultural backgrounds.
5. Experience working with chronically homeless individuals
6. Bilingual (English/Spanish) preferred
