Are you the Transition Coordinator we have been looking for? In this position, you will be able to convey your passion for helping others with a dynamic team by bringing an energetic and positive approach while working with youth that we serve!
Cohannet Academy is an Intensive Residential Treatment Program for adolescents that specializes in trauma informed care. Cohannet provides 24-hour comprehensive care including a school on site, a full clinical staff, nursing staff, and direct care staff.
Compensation
The pay range for this position is $22.00 to $30.00 per hour. The listed range reflects what we reasonably and in good faith expect to pay for this role at the time of posting the position. The actual compensation an employee is offered may vary based on several factors such as experience, education, licensure, certifications and qualifications. This range may be modified in the future.
Due to the nature of our work and the youth who are depending on us, there are times where overtime may be mandated.
What You'll Do:
- Provide comprehensive support to residents getting ready to transition to their next steps.
- Assist residents in mastering competency in their abilities to move forward.
- Support residents as they build stronger relationships in the community and at home.
- Help residents discover their interests and build hobbies within their community.

Requirements:
- Must be 21 years of age.
- Bachelor's degree preferred.
- COVID-19 Vaccination required.
- Maintains valid driver’s license as driving residents to home for visits and to community activities is required.
- Available to work evenings, holidays, weekend shifts as required.
- At JRI, you will learn to provide physical interventions and verbal de-escalations to provide the safest therapeutic atmosphere for youth that we serve!
- Background and driving record checks will be performed.
