Just Vital Solutions PLLC empowers healthcare providers to deliver high‑quality, value‑based care through innovative Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) services. We support practices by providing FDA‑approved devices, technology platforms, and operational support that improve outcomes for high‑risk patients while reducing hospital readmissions and overall healthcare costs.

We are seeking an experienced Medical Assistant to support our growing RPM and CCM programs.

This role is ideal for a Medical Assistant with experience in RPM and/or CCM who enjoys patient engagement, technology‑enabled care, and proactive care coordination. You will play a key role in monitoring patient data, supporting chronic care initiatives, and collaborating with providers to ensure timely interventions and high‑quality virtual care.

• Review and monitor patient vital sign data from RPM devices (blood pressure, glucose, pulse oximetry, weight, etc.)
• Identify abnormal trends and escalate findings to providers per established clinical protocols
• Conduct CCM patient outreach, monthly check‑ins, and patient education
• Assist patients with RPM device onboarding, troubleshooting, and ongoing engagement
• Accurately document patient interactions, vitals, and care coordination activities
• Ensure CCM time tracking, documentation, and compliance requirements are met
• Collaborate with providers and care teams to improve patient outcomes
• Maintain accurate and organized patient records within telehealth platforms and EHRs
• Collaborate on-site with providers when we implement a contract and ensure that the practice has a supply of devices and assist in on site supply management and help distribute devices to eligible patients in real time.

• Certified Medical Assistant (CMA, RMA, or equivalent) preferred
• Prior experience with Remote Patient Monitoring (RPM) and/or Chronic Care Management (CCM) strongly preferred
• Comfort using medical devices, telehealth platforms, and mobile applications
• Strong communication skills with a patient‑centered approach
• Excellent documentation and organizational skills
• Ability to work independently in a hybrid/remote environment
• Familiarity with telehealth workflows and value‑based care models is a plus

• $19.00 – $25.00 per hour, based on experience
• Compensation is commensurate with prior RPM and CCM experience
• Part‑time hours with flexibility
• Hybrid role (remote work with occasional in‑office support in The Woodlands)

Candidates with hands‑on RPM and CCM experience are typically hired toward the top of the range.
