Job Summary
We are seeking a dedicated and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong background in patient care, with experience across various medical settings including acute care, long-term care, and outpatient services. This role involves providing high-quality nursing care, performing assessments, administering treatments, and supporting patient recovery and well-being. Candidates with expertise in EMR/EHR systems, medical documentation, and specialized procedures such as phlebotomy, suturing, and catheterization are highly valued. Duties
- Conduct comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations.
- Administer medications and injections accurately following HIPAA guidelines and aseptic techniques.
- Document all patient interactions and treatments using ClinicalWorks.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Provide specialized care for diverse populations including pediatrics, geriatrics, patients with developmental disabilities, dementia or Alzheimer's disease, hospice & palliative medicine patients.
- Perform diagnostic evaluations such as laboratory specimen collection/processing and imaging procedures like sonography or mammography when applicable.
- Support discharge planning and case management activities to ensure smooth transitions of care across settings.
- Maintain infection control standards and sterile processing protocols to ensure patient safety.
- Educate patients and families on health coaching, medication administration, nutrition, pain management, and disease prevention strategies.
- Chronic Care Management coordination creating care plans and reaching out to offices.
Requirements
- Current licensure as an RN or LPN in the state of practice.
- Proficiency with eClinicalworks.
- Strong knowledge of medical terminology, anatomy & physiology, ICD coding (ICD-9/ICD-10), CPT coding, and diagnostic evaluation techniques.
- Experience with advanced procedures such as suturing, ostomy care, dermal fillers (nice-to-have), or esthetic laser treatments is advantageous.
- Ability to work effectively with diverse patient populations including children (pediatric care), elderly (geriatrics), individuals with disabilities or memory care needs.
- Demonstrated competence in critical care environments such as ICU or trauma centers; experience with ventilators or neonatal ventilators is preferred.
- Excellent communication skills for patient education and interdisciplinary collaboration.
- Ability to adhere to infection control protocols and maintain sterile technique during procedures.
