**Required Experience:**
- 1 years of PT (Physical Therapy) Experience
- Outpatient Experience

- Active Illinois/IL Physical Therapist License
- Active American Heart Association BLS (Basic Life Support)

**Required Skills and Qualifications:**
- Strong experience administering a wide range of physical therapy techniques
- Effective communication and strong interpersonal skills

**Submission Requirements:**
- Candidates' Permanent Address (Facility will not consider local candidates - 50-mile radius restriction)
- Work history
- Skills Checklist, current to 1 year, pertinent to the specialty (Physical Therapy)
- 2 Supervisory Reference Forms Completed with dates employed listed

**Certifications:**
- BLS (American Heart Association)

**State Licenses:**
- Illinois
