**Required Experience and Qualifications:**

- 2 years of previous experience
- At least 1 travel contract completed
- Epic experience
- Not an employee of UnityPoint in the last 12 months
- Driver's License
- Work history/resume
- Skills checklist completed within 1 year of submission date
- License verification
- Copy of required certifications
- 1 supervisory reference required at the time of submission
- All candidates must be at least 50 miles from the facility (based on driver's license)

**Clinical and Professional Skills:**

- Contributes professional/clinical knowledge and skills in the provision and management of care to patients.

**Facilities Information:**

- Madison, Sioux City, Bettendorf, and Fort Dodge: Level 3 trauma center
- Moline: Level 2 trauma center
- Muscatine: Level 4 trauma center
- Rock Island: Level 1 trauma center
