Job Summary
We are seeking a dynamic and compassionate Case Coordinator to join our dedicated healthcare team. Responsibilities
- Conduct thorough patient assessments to determine needs related to medical, behavioral health, social determinants, and discharge planning.
- Collaborate with healthcare providers across hospital departments such as ICU, emergency medicine, pediatrics, and behavioral health to develop individualized care plans.
- Manage inpatient and outpatient cases by coordinating services including post-acute care, home health, rehabilitation facilities, and community resources.
- Facilitate communication between patients, families, insurance providers, and multidisciplinary teams to ensure optimal care delivery.
- Perform utilization management activities by reviewing medical records and supporting appropriate resource utilization in compliance with HIPAA regulations.
- Assist with diagnostic evaluations, medical records documentation, and patient intake processes to support accurate case documentation.
- Support discharge planning efforts by organizing necessary services and ensuring smooth transitions from hospital to post-acute or outpatient settings.
Requirements
- Proven experience in hospital environments such as Level I or Level II trauma centers with acute care exposure.
- Strong background in hospital medicine, ICU or emergency medicine settings preferred.
- Knowledge of medical terminology, physiology, and patient assessment techniques essential for effective case management.
- Experience working with behavioral health or mental health counseling in inpatient or outpatient contexts is highly desirable.
- Familiarity with managed care processes, utilization management protocols, and diagnostic evaluation procedures.
- Excellent communication skills to coordinate across multidisciplinary teams including social work, addiction counseling, pediatrics, and post-acute providers.
- Ability to navigate electronic medical records (EMR), maintain HIPAA compliance, and handle sensitive patient information accurately.
