JOB DESCRIPTION: CASE MANAGER for (Baltimore City, Cecil County, Prince George's County)
Objective: To meet the daily life needs of clients. To act as a liaison between Jumoke, Inc., the placing agency, and parents/guardians. To provide optimal services while promoting the company.
Requirements: This is a full-time position requiring a B.S. or B.A. degree in the human services field and at least 2-3 years of experience providing case management. The case manager must be able to work and succeed in a fast-paced environment, work well independently, and work well under the supervision of our Clinical team.
Responsibilities:
- Act as the liaison between Jumoke, Inc., the placing agency, and parents/guardians in the areas of admission, discharge, progress, concerns, or critical incidents involving clients
- Promote Jumoke, Inc.’s services and solicit referrals to the program
- Assist in determining the appropriate service and/or facility placement for client admission
- Assist with the orientation of new clients to the program and/or facility
- Participate and assist in the development and execution of the client’s treatment plan
- Assist in maintaining the health, safety, and rights of the client and staff
- Prepare referrals, progress reports, transfer summaries, treatment plans, assessments, school referrals, Critical Incident Reports, monthly summaries, intake and discharge reports [APPENDIX]
- Represent the client at ARD/IEP meetings or any other applicable meetings
- Assist with organizing and implementing activities for clients when appropriate
- Determine and recommend client readiness for discharge
- Monitor appropriateness of disciplinary action
- Schedule home visits, special visits, medical appointments, and educational placement for clients
- Monitor special visits of clients, e.g. with parent(s), police, or attorney, when appropriate
- Arrange and ensure posting of the client’s visitation outside the facility and list all visitors
- Maintain accurate, updated, and confidential client records, including signatures on 808 Forms and 818 Forms
- Review for accuracy all documentation pertaining to clients, e.g., facility logbook, tracking notes, etc.
- Report abuse or neglect of clients
- Communicate regularly with Residential Supervisors, Counselors, Clinical Staff, and Administrative Staff, and keep abreast of client matters
- Any other duties deemed necessary by the Executive team
- A valid driver’s license is required.
- The candidate must possess the ability to pass a CPS, state, and federal background check upon hiring.
- The candidate must possess the capability to maintain a regular timetable during the week and on holidays when necessary.
- Full-time: Monday-Friday, 9:00 am-5:00 pm, and some off days and hours.
