Duties
- Perform comprehensive patient assessments including vital signs, physical examinations, and health history documentation using manual note taking forms.
- Administer medications via feeding tube, following strict protocols and HIPAA regulations to ensure patient safety.
- Provide wound care (feeding tube stoma), Tube Feeding Management using an Infinity Pump System, Nebulizer Treatments, CPAP Therapy, CPT Therapy, PRN Oxygen Therapy.
- Educate patient and their Caregivers on disease management, medication administration, nutrition, and wound care techniques to promote optimal health outcomes.
- Document all patient interactions using manual note taking forms while maintaining compliance with HIPAA standards.
- Collaborate with interdisciplinary teams including physicians, physical therapists, social workers, and community resources to develop comprehensive care plans.
Experience
- Valid Licensed Practical Nurse (LVN) license with current state registration.
- Recent graduate or limited experience in home health nursing, long care term facilities, or nursing home will be considered, although proven experience in home health nursing is preferred.
- Background working with diverse patient populations including geriatrics, pediatrics, individuals with disabilities.
