About This Opportunity
Judit Inc. is hiring on behalf of one of our healthcare clients, an accredited home health care agency. They provide skilled nursing, rehabilitation, post-surgical, chronic and pediatric care to patients in their homes across the New York City area. This is a temporary, contract position: you'll be employed by Judit Inc. and work directly alongside our client's clinical team.

Home Care services are provided in all 5 NYC boroughs, Nassau and Suffolk Counties
What you'll do
As a Home Care RN, you'll plan, organize and direct skilled nursing care for patients in their own homes. You'll assess each patient and family and build the plan of care. You'll also draw on community resources to help patients recover and stay safely at home.
- Complete an initial assessment of the patient and family to determine home care needs
- Perform a full physical assessment and history of current and previous illnesses
- Use assessment data to determine nursing diagnoses
- Develop a care plan with goals based on the nursing diagnosis, including therapeutic, preventive and rehabilitative nursing actions
- Involve the patient and family in the planning process
- Initiate the plan of care and revise it as the patient's status and needs change
- Regularly re-evaluate patient nursing needs
- Counsel patients and families on nursing and related needs, and provide health instruction based on the assessment and plan of care
- Identify discharge planning needs during care plan development, before the patient is discharged
- Serve as RN Case Manager when assigned by the Clinical Supervisor, coordinating care for your assigned caseload
What you'll bring
- Active New York State RN license and current registration
- 1 year of medical/surgical nursing experience
Nice to have
- Associate or Bachelor's degree in Nursing from an approved program
- Experience with individuals with intellectual and developmental disabilities (MRDD) and/or prior home care experience
- UAS-NY certification
- Early-career nurses are encouraged to apply
Work details
- Visits take place in patients' homes across the New York City area, with travel between visits
- You'll need to travel between homes and use stairs or walk-up buildings, with or without reasonable accommodation
