You will be responsible for providing comprehensive physical therapy services to homebound patients as part of an interdisciplinary team.
- Conduct initial and ongoing comprehensive assessments of the patient's functional status, including mobility, strength, and home environment safety.
- Develop, implement, and document the Plan of Care (POC) in coordination with the physician and clinical supervisor.
- Provide skilled physical therapy treatments such as therapeutic exercises, manual therapy, and gait training to restore function and prevent disability.
- Educate patients, family members, and caregivers on the safe and effective use of necessary medical equipment, home exercise programs, and safety precautions.
- Complete all clinical documentation accurately and efficiently using our Electronic Medical Record (EMR) system.
- License: Current and unrestricted Physical Therapist (PT) license in the State of California.
- Experience: 1 year of clinical experience as a Physical Therapist (Home Health experience strongly preferred).
- Certification: Current CPR certification (AHA Basic Life Support required).
- Logistics: Valid driver's license, reliable and insured vehicle, and comfortable with travel across the East County service area.
- Skills: Strong clinical judgment, excellent communication, and a compassionate, patient-first attitude.
