What are the responsibilities and job description for the Hospice Triage LPN position at JSSA?
JSSA Hospice is looking for a Triage nurse to assist patients and their families during the end-of-life process.
Responsibilities:
Support primary nurses and doctors with handling clinical and scheduling/administrative hospice calls regarding patient care
Oversee the scheduling and productivity of the hospice aides (CNAs/CMTs)
Provide administrative support in preparing for and during IDG meetings
Assist with any office administrative tasks as needed
Attend all required meetings
Available to work onsite Monday through Friday from 7:30am-4pm
Able to make filed visits to perform annual and as-needed hospice aide supervisory visits
Qualifications:
Active LPN license to practice in Maryland
3-5 years’ experience performing related work
Demonstrated organizational and time management skills
The ability to multitask and work in a fast-paced environment
Demonstrated flexibility, adaptability, and outstanding customer service skills
Proficient with MS Office 365(Teams, Word, Excel), Electronic Medical Records and other databases
Familiarity and comfort with hospice medical terminology
Ability to manage confidential information with sensitivity
Outstanding phone and in person customer service skills
