Job Summary: The Manager, Inpatient Case Management oversees the development, implementation and evaluation of day to day operations of the inpatient case management team responsible for the functions of discharge planning and case management services. This role serves as a liaison for the interdisciplinary team, payers, physician group and various agencies related to case management activities.
Essential Job Functions & Accountabilities:
- Ensures the units are adequately, completely and competently staffed in accordance with budgeted plan; and, personnel are appropriately trained and supervised.
- Responsible for selecting, hiring, and terminating personnel within the department. Evaluates personnel work performance, counsels staff, and initiated disciplinary action as appropriate.
- Ensures interventions are in compliance with known legal and regulatory requirements.
- Evaluates the daily tasks and interventions provided by the inpatient case management staff. Rounds for outcomes on patients, staff and providers.
- Acts as a liaison between the Director and others within the organization for decision making and problem- solving related to care facilitation, case management, community resources utilization, patient education, etc.
- Intervenes in the plan of care when necessary of complex cases requiring coordination of medically related care and leverages the Physician Advisor as needed.
- Collaborates with practitioners, nurses, clinical managers, various agencies and other members of the health care team to select, guide and direct patient care as it relates to discharge planning and the CMS C.O.P. Initiates and participates in collaborative efforts that provide on-going evaluation of program operation, compliance and goal attainment.
- Assists in the preparation of the budget. Maintains accountability to include adequate and appropriate staffing patterns, supplies, equipment and physical facilities to meet the patients' needs.
- Develops, recommends, implements, reviews and revises appropriate departmental policies and procedures.
- Maintains accurate statistical data and assists with developing department reports.
- Provides on-going education and training to the case management guidance and recommendations to staff regarding resource utilizations thereby contributing to cost-effectiveness/efficiency.
- Functions as a case manager during staff shortages.
- Serves on committees representing Case Management.
