Job Summary: The Licensed Clinical Therapist is responsible for providing assessments and therapeutic services to a broad range of patients in the appropriate modalities. Utilizes their subject matter expertise to provide mental health and substance abuse counseling to Behavior Health patients. This position functions as a member of the treatment team, providing direct care and other support services in a way that promotes positive treatment outcomes and meets the needs of our behavioral health patients.
Essential Job Functions & Accountabilities:
- Identifies and quickly and accurately provides clinical assessments of mental health and substance abuse conditions.
- Utilizes behavioral health knowledge and expertise to promote change and improve behavioral and physical health conditions.
- Conducts brief psychotherapy individual and/or group therapy sessions using evidenced based modalities and interventions.
- Provide care management skills and knowledge of local resources for outside referrals.
- For shared patients, assists the care team in recognizing, treating and managing mental health and psychosocial issues.
- Documents session content, treatment goals and plans in the electronic medical records system.
- Develops relapse prevention plans and assists clients with maintaining and functioning according to the plan.
- Provide clients with self-management skills and educational information so that they can be a full participant in their own treatment and recovery.
- Participates in treatment planning for assigned patients.
- Charts assigned patients per operating procedure and maintains other records as assigned.
- When appropriate, liaisons with patient’s family or other support systems regarding the patient’s care, treatment plan, and discharge plans.
- Assumes responsibility for crisis counseling, psycho-educational services, or other responsibilities as assigned.
- Assumes primary responsibility for discharge planning, including assessment of needs, coordinating referrals, consulting with other service providers, and consultation with other community resources, as appropriate.
- Maintains current knowledge about issues affecting the adolescent, adult and geriatric population.
- Maintains current community resource information regarding services for adolescent, adult and geriatric patients.
- Maintains clinical competence for adolescent, adult and geriatric services and exhibits professional growth and development.
- Utilizes behavioral health expertise to promote change to improve behavioral and physical health conditions.
- Ensures compliance with patient HIV pre-counseling and post-counseling per policy.
