Job Summary: The Centering Pregnancy Program Coordinator is responsible for understanding the 13 essential elements of forming Centering Pregnancy groups as indicated by the Centering Healthcare Institute. This role will facilitate recruitment of patients, group formation, organization of support services and materials, and data tracking of Centering statistics related to individual sessions and group trends. This position requires analysis on challenges and opportunities for improvement in all aspects of the program. The Coordinator will also be involved in the annual accreditation renewals and any site approvals.
Essential Job Functions & Accountabilities:
- Coordinates dates and locations for groups and centering pregnancy scheduling. Coordinates room scheduling at specific clinic locations (JPS Health Center for Women Main, JPS Health Center for Women Northwest and/or JPS Health Center for Women Southeast).
- Coordinates co-facilitators with providers for group assignments.
- Assists with templates by requesting changes and ensuring accuracy between EPIC and the Provider’s schedule. Continuously engages in concerted action with schedulers to communicate any changes in provider schedules at each site.
- Coordinates Provider, Staff schedules and patient visits.
- Initiates communication within the OB/GYN practice to promote new and existing centering cohorts.
- Utilizes a collaborative care team approach to accomplish goals.
- Participates in patient enrollment activities for the Centering Program.
- Responds to patient inquiries and concerns about Centering Pregnancy Group via phone calls or Epic messages.
- Assists in developing and updating promotional material (posters, flyers, website content, social media, etc.).
- Plans group activities with facilitators, maintains inventory and stocking of medical, office, and centering class supplies.
- Ensures ideal group size, monitors and maintains group census.
- Drafts and delivers Bi-Monthly Centering Update Newsletter to department.
- Participates as a regularly scheduled co-facilitator for Centering sessions and provides co-facilitator staffing as needed.
- Operates and uses medical equipment including BP Cuff, scales, prenatal wheels and calendars, computer electronic medical record.
- Provides patient/family instruction in preventive health activities and interventions according to protocol, as needed under the direction of the nursing leadership or medical provider.
- Participates in annual In-Service Trainings with staff, nurses, midwives, and physicians.
- Tracks data of Centering statistics related to individual sessions and group trends.
- Assists in the annual accreditation renewals and any site approvals.
- Participates in patient care quality improvement projects.
