Salary: $56,680.00 - $70,850.00Job SummaryAFamily Resources Coordinator works with familiesand caregiverswith children ages birth to three yearswho have been referred due toconcerns fordevelopmental delays.This position supports familiesand caregiversin the coordination of services and resources throughout their time in JoyasEarlySupportServices(ESS).The individual in this position has a passion for helping children and their families/caregivers.Duties and Responsibilities:Completeinitialintakeswithfamiliesand caregiverseitherin-person, virtuallyorbyphoneasdirectedbyNurse Enrollment Coordinator.Act as a social service coordinator for families/multidisciplinaryteams.Assistnew families in applying for services such as Developmental Disabilities Administration(DDA),Supplemental Security Income (SSI), Apple Health (Medicaid) asindicated.Act as agency contact person for multi-agencymeetings.Provide in-home, virtual or phoneconsultationsin conjunction with Joyamultidisciplinary team members.Developandfacilitateparent supportgroupsand/oreducationalgroups asagreedupon with theProgram Director.ParticipateintheMedicaidAdministrativeClaiming(MAC)programandprovidesupporting documentation and timesheets for "matchable"activities.Act as a Family ResourcesCoordinator within theEarly Supports for Infants and Toddlersprogram (ESIT).Collaborate with families, teachers and therapists to identify strengths and needs of the childandfamilyinordertocoordinateanddevelopanIndividualizedFamilyService Plan (IFSP).Develop IndividualizedFamilyServicePlan(IFSP)andenterintotheEarlySupportfor Infants and Toddlers (ESIT) Data Management System(DMS).Coordinate transitions to schooldistricts.Work with families of all socioeconomic incomes, including outreach activitiesand carecoordination for Medicaid and Medicaid eligiblefamilies.Perform other related duties as assigned.Supervisory Responsibility:NoneRequired Skills & Abilities:Thorough knowledge of community social servicesystems.Cultivate positive,supportiveand responsive interactions while encouraging independence.Exhibitscompassionwhile incorporating the individual and cultural differences of families.Act in a respectful manner toward each person regardless of age, gender, race, nationality, religion, ethnicity, social or economic status, sexual orientation, health condition or disability.Ability to communicate effectively and make independentdecisions.Ability tomaintainconfidentiality of sensitive information.Understanding of and ability to resolve conflict.Knowledge of HIPAAand FERPArequirements/regulations.Excellent organizational skills and attention to detail.Proficient in Microsoft Office Suite.Education, Certifications and Position Requirements:Bachelor'sdegreein ahealthoreducationrelatedfield(e.g.,healtheducation,social work, public health, special education, early childhood education ornursing).Masters degree in social workor RN licensepreferred.Two to three years of experience in working with families,case management, social work/social welfare, infant mental health, early childhood development, orcounseling preferred.Current registrationwiththeWAStateEarlySupportforInfantandToddlers (ESIT)EarlySupport Services (ESS)or ability to complete the requirements within three months of employment.Valid driver's license and vehicle with proof of insurance.First Aid / CPRCertificateBloodborne Pathogens Training(will be provided).T.B.Test, proof ofTDapImmunization,Hepatitis BvaccinationsWashington State Department of Early Learning Criminal backgroundclearance.DSHS Background Check clearance.Environmental Factors:Ability to lift or carry up to 40 poundsregularly.Indoors,sedentaryupto7hours/daysittingorstandingandupto4hours/day keyboarding.Abilitytonavigateclientsites(unpavedwalkways,stairs,narrowhallways,etc.)ona regularbasis.Ability to bend, twist, stoop on a regularbasis.Disclaimer:This job description is not designed to cover orcontaina comprehensive listing of activities, duties and responsibilities that arerequired ofthe employee.Duties,responsibilitiesand activities maychangeor new ones may be assigned at any time with or without notice.
