- Position type: PRN (as-needed)
- Schedule: Night shift, 7:00 PM – 7:30 AM
- Minimum requirement: 2 shifts per month (additional shifts available and preferred)
- Pay: Up to $46.63/hour - offer based on experience
- Shift differentials: $2.50/hour (weeknights), $5.00/hour (weekend nights)

- Provide skilled hospice nursing care, including patient assessment, admissions, and care planning
- Manage a hospice caseload and coordinate care with the interdisciplinary team
- Assess physical, emotional, social, and spiritual needs of patients and families
- Communicate with physicians and care teams to ensure continuity of care
- Perform nursing interventions and follow physician-directed treatment plans
- Conduct follow-up visits and update patient care plans as needed
- Provide end-of-life care, including clinical pronouncement and family support
- Maintain accurate, timely documentation in compliance with regulatory standards
- Hospice experience required (applicants without hospice experience will not be considered)
- Active Registered Nurse (RN) license in Illinois
- Graduate of an accredited nursing program
- Current CPR certification
- Valid driver’s license and auto insurance
- Strong communication and interpersonal skills
