JourneyCare Hospice is hiring a Registered Nurse Clinical Liaison to support patients and families at Northwestern Memorial Hospital as they transition to hospice care.
In this role, you'll use your clinical expertise and communication skills to guide patients and families through care options, provide bedside hospice evaluations, and help ensure a smooth transition from hospital to hospice services.

Key Responsibilities
- Respond to hospice referrals and complete bedside assessments
- Educate patients and families on hospice services and benefits
- Assist with patient intake and admissions process
- Evaluate patients for hospice eligibility
- Coordinate care with physicians, hospital case managers, and interdisciplinary teams
- Facilitate General Inpatient (GIP) admissions as needed
- Serve as an on-site clinical resource for hospice transitions
- Maintain strong communication with internal and hospital teams
- Ensure all admission documentation is completed accurately
Qualifications
- Graduate of an accredited RN program
- Active Illinois RN license
- Current CPR & First Aid Certification
- 1 year of nursing experience (preferred)
- Ability to travel 50% or more within assigned territory
- Strong clinical judgment and interpersonal skills
- Valid drivers license and proof of insurance required
- Knowledge of quality and compliance procedures
