Job Summary:
JourneyCare Hospice is seeking a Hospice LPN to join our growing Crisis Care team.
This position requires following the plan of care for our continuous care patients, providing treatments, and working collaboratively to help our patients and their families.

Responsibilities:
- Provide direct patient care as defined in State Nurse Practice Act.
- Implement current nursing practice following a comprehensive assessment and the plan of care.
- Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
- Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
- Provide appropriate pain/symptom management.
Salary : $137,440 - $199,200
