Job Description:
Are you passionate about making a positive impact in the lives of individuals facing challenges and in need of support? We are looking for a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in providing guidance, coordination, and advocacy to help our clients achieve their goals and improve their overall well-being.
Responsibilities:
- Conduct comprehensive assessments to understand clients' needs, strengths, and challenges.
- Develop individualized case plans in collaboration with clients, setting achievable goals and action steps.
- Provide support, resources, and referrals to assist clients in accessing essential services and programs.
- Monitor and evaluate clients' progress and adjust case plans as necessary.
- Advocate for clients' rights and needs, ensuring they receive the best possible care and services.
- Maintain accurate and up-to-date case records and documentation in compliance with agency standards.
- Collaborate with a multidisciplinary team to coordinate client care and support.
Qualifications:
- Bachelor's or Master's degree in Social Work, Psychology, Counseling, or a related field.
- Relevant licensure or certification is a plus but not required.
- Strong communication and interpersonal skills.
- Empathy, patience, and a client-centered approach to service.
- Organizational and time-management skills.
- Ability to work effectively in a team-oriented environment.
- Knowledge of community resources and social services is advantageous.
