Job Summary

The Community Support Specialist provides direct assistance and supportive services to individuals and families in the community. This role focuses on helping clients access resources, navigate systems, and improve overall well-being through advocacy, education, and care coordination. The specialist works closely with clients to address barriers and connect them to essential services such as healthcare, housing, employment, and social supports. This role supports individuals by addressing barriers to care, coordinating services, and ensuring clients can access necessary benefits and healthcare resources. The Community Support Specialist is accountable for all related clinical, administrative, and quality and compliance responsibilities.

Schedule And Compensation

  • Monday - Friday 9 AM - 5 PM
  • $42,000 - $50,000 based on experience

Essential Responsibilities And Metrics

  • Assess client needs and develop individualized service or support plans while providing direct client care 50% of the time
  • Plan, develop, and facilitate structured skills-building groups (e.g., life skills, communication, coping strategies, employment readiness, daily living skills)
  • Adapt group content to meet participants’ needs, learning styles, and abilities
  • Connect clients with community resources, including healthcare, housing, food assistance, and social services
  • Assist clients with Medicaid enrollment, renewal, and eligibility processes
  • Educate clients on Medicaid benefits, coverage options, and how to use their insurance
  • Support clients in identifying healthcare providers and scheduling appointments
  • Maintain accurate and timely documentation in accordance with organizational standards
  • Monitor client cases and provide follow-up to ensure successful outcomes
  • Advocate on behalf of clients to ensure access to appropriate services and supports
  • Assist clients with executive functioning skills and independent living skills
  • Attendance and participation at clinical presentations
  • Use of supervision or consultation when required or needed

Qualifications

  • Clinically related bachelor’s degree in psychology, counseling, or human service
  • Experience working in community-based services, case management or human services preferred
  • Familiarity with public assistance programs preferred i.e. Medicaid, SNAP, SSDI,
  • Strong interpersonal and communication skills
  • Ability to build trust and rapport with diverse populations
  • Demonstrated fluency in English and Spanish, including speaking, reading, and writing at a professional level; required
