Key Responsibilities
- Provide comprehensive primary care (assessment, diagnosis, treatment, follow-up)
- Order and interpret labs and diagnostic tests
- Perform medication reconciliation and promote safe prescribing
- Develop individualized care plans
- Educate patients and families on disease prevention and chronic disease management
- Collaborate with physicians and interdisciplinary care teams
- Maintain accurate and compliant documentation
- Participate in quality improvement initiatives
Requirements
- Active Nurse Practitioner license
- Active RN license
- Prescriptive authority (or ability to obtain)
- Experience with adult and geriatric populations preferred
- Spanish fluency required
- Strong communication and teamwork skills
- Commitment to serving underserved communities
