- CLASS A CDL
- NO MORE THAN 1 MOVING VIOLATION IN THE PAST YEAR
- NO MORE THAN 1 PREVENTABLE IN THE PAST YEAR
- NO DUI, DWI, ETC. IN THE PAST 5 YRS.
