Position: Targeted Case Manager
Job Summary
As a Targeted Case Manager, you will manage a moderate-sized caseload, utilizing tools to complete tasks efficiently and effectively. Your role will involve scheduling and facilitating individual support planning, completing assessments and screenings, writing Individual Support Plans and Addendums, and ensuring all necessary documents are complete and signed. You will also coordinate cases, link individuals to additional resources, and maintain regular contact with those you serve.
Key Responsibilities
- Schedule and facilitate Individual Support Plan meetings.
- Complete Individual Support Plan Monitoring and Reviews.
- Make referrals for Due Process.
- Write Individual Support Plans and Addendums.
- Ensure annual documents are complete with signatures.
- Complete budget reauthorizations and utilization review packets to fund necessary support for individuals we serve.
- Coordinate cases and link individuals to additional resources.
- Maintain contact with individuals per DMH guidelines.
- Achieve a minimum of 345 direct Medicaid billable hours per quarter.

- Basic computer knowledge, including Microsoft Office and Adobe.
- Proficient math skills.
- Ability to drive independently to various locations and have reliable transportation.
Education and Experience
- Bachelor’s degree required.
Physical and Mental Requirements
- Ability to remain stationary 80% of the time.
- Occasionally move about the office to access supplies.
- Use repetitive motions for prolonged periods.
- Occasionally move packages and office supplies up to 25 pounds.
- Meet multiple demands and interact with the public and staff.
