Summary:

The Physician Liaison/Record Integrity Analyst, under the leadership of the HIM Supervisor/Manager, performs detailed qualitative and quantitative analysis of hospital medical records to ensure completion and compliance with hospital policy and state and federal regulatory agencies and to ensure timely documentation if available for coding and billing purposes. This position is responsible for performing clinical pertinence audits, auditing record content against Medical Staff Rules and Regulations, The Joint Commission Standards (TJC), and CMS regulations. The results of these audits will be reported to Medical Staff committees in each hospital.

Essential Functions:

  • Review, identify and assign documentation deficiencies to the correct provider(s).
  • Continually monitor hospital record completion and ambulatory encounter compliance.
  • Serve as a resource to physicians and other providers regarding the need for timely and complete documentation for coding, TJC and CMS purposes
  • Serve as the primary contact person for providers investigating record completion issues for incorrect deficiency and/or patient class assignments.
  • Collaborate with hospital departments, including Patient Financial Services and Registration to resolve documentation issues.
  • Communicate with providers on a regular basis regarding incomplete documentation, including unanswered coding queries, and escalate to physician and hospital leadership according to policy.
  • Implement physician suspension procedures when records are not completed within the required time frame and notify impacted departments of the suspension.
  • Reconcile deficient clinical documentation reports utilizing excel spreadsheets.
  • Identify incorrect note types and perform note type chart corrections.
  • Review and manage the verbal/co-sign orders according to hospital policy.
  • Locate, track and route problematic provider dictations to the correct patient record within the EHR.

Education:

  • Associates Degree or active enrollment in an accredited Health Information Management Program.
  • For internal candidates, Associate’s Degree preferred or two years of HIM experience and has exceeded the JHHS quality and productivity standards for the previous six months.

Required Licensure, Certification, On-going Training:

  • None. RHIT preferred.

Work Experience:

  • Track One: Two years Health Information Management department or clinical setting required.
  • Track Two: RHIT/RHIA credential with one-year Health Information Management experience required.

Salary Range: Minimum 24.32/hour - Maximum 40.16/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
