Job Summary: The Care Coordinator is responsible for overseeing the delivery of all resident care while maintaining compliance with state, federal and JKV expectations and guidelines at all times. Care delivery will be tailored to fit each resident and to reflect approaches best for those we serve. The Care Coordinator is responsible for the overall clinical excellence of our assisted living community. Main areas of focus and oversight include the following: care management, medication management, risk management and quality assurance, and staff management. A focus on customer service and resident and family relations are critical.

- Supervise the overall implementation, delivery and coordination of resident care and services.
- Oversee the care and services provided by the nursing staff. Plan, assign and direct clinical responsibilities and tasks.
- Observe and provide accurate and timely evaluation of residents for care needs. Promptly report any changes to the Administrator.
- Review resident care, recommend changes and implement changes when needed.
- Ensure resident care interventions are according to physician orders and service plans to meet the personal, physical, social, cognitive and psychological needs of residents.
- Provide oversight of medication management procedures to ensure documentation is accurate and thorough on the EMR/MAR.
- Ensure all resident documentation is performed accurately and according to state and federal regulations.
- Coordinate admissions, transfers and discharges.
- Provide direction and supervision to licensed nurses, personal care aides, certified nursing assistants and medication technicians.
- Review and report all incidents and unusual occurrences for residents, staff or visitors in a timely manner and conduct proper follow up.
- Assure notification of physician, family and Administrator when a resident becomes ill or is injured.
- Participate in case conferences, in-services and staff meetings.
- Attend to resident complaints and requests, including answering them, required documentation and reporting to supervisor and the Administrator. Promote healthy relationships with resident, families and caregiver.
- Give direct nursing care, utilizing skill and judgment, as prescribed by the physician within the scope of practice and the rules and regulations for assisted living facilities.
- Keep Administrator informed of any and all problems at all times.
- Cooperate with other departments and support staff in the delivery of care and services to the residents.
- Adhere to all Oakview Suites policies and procedures.
- Adhere to all John Knox Village policies and procedures
- Adhere to all John Knox Village rules such as driving, parking and smoking.

- Usually works indoors but may have to work outside during outside events and parties.
- May have to get into awkward positions or work in cramped spaces.
- May be exposed to loud noises or sounds that are distracting.
- May be exposed to diseases or infections from contact with residents or other staff members.
- Often exposed to contaminants.
- May sometimes need to wear safety gear such as gloves and or masks.
- May have to handle hazardous waste in accordance with infections control procedures.

- Employee must be able to perform the essential functions of the job with or without reasonable accommodations. - For the safety and security of our residents and patients, employees must be able to read, write, and verbally communicate in English, which is the most universally used language of our resident and employee population.
- Routinely provides assistance to residents and or other staff members which could require lifting, pushing, pulling or balancing the equivalent of 50 pounds or greater at times.
- Must be capable of maintaining regular attendance and reporting to work on time.
- May be required to work nights, weekends or holidays. Needs to be flexible to work other shifts or perform other duties as assigned.
- Maintain a current active Florida nursing license. Provide John Knox Village with current license upon renewal of license.
- Maintain current CPR certification.
- Must keep all required CEU’s up to date for nursing license renewal.
- Complete and maintain all assisted living specific state required trainings; ie.CORE Training
- Must maintain a professional, respectful, friendly demeanor with all residents, family members, guests, and staff at all times.
- Perform Resident Nurse duties after hours and on weekends.
