We are looking for a compassionate Patient Care Coordinator to join our Long Term Care team. We offer a competitive salary and benefit package.
Shift:
- Full-Time - 8a-4:30p (Days)
- 80 hours every two weeks
Principle Accountabilities and Essential Duties of the Job:
- Assesses patient needs on admission and throughout the healthcare continuum and documents accordingly.
- Plans patient care in collaboration with members of healthcare team.
- Coordinates/assists in implementation of the patient's plan of care to attain optimal level of wellness in the most cost effective manner and documents accordingly.
- Evaluates the care delivered and intervenes appropriately to ensure optimal patient outcome and documents accordingly.
- Exhibits customer-focused behaviors which are consistent with the mission, vision, values statements and the Patient's Bill of Rights.
- Communicates patient information to ensure continuity of patient care.
Candidate needs to have the following:
- Valid Missouri LPN license - Preferred
- 2-3 years of experience - Preferred
- Excellent bedside manner and communication skills
- Reputable work ethic
- Highly motivated and driven to provide exceptional care
