We are currently seeking an exceptional, experienced, and dedicated Registered Nurse (RN) for our Home Health division. This is a full-time, permanent position that operates from Monday through Friday, allowing you to regain your work-life balance. As part of our team, you will play a pivotal role in providing high-quality patient care in the comfort of their homes. This position is ideal for a compassionate, patient-focused professional who is committed to providing superior care and comfort to patients and their families.
Responsibilities:
As a Home Health RN, you will be entrusted with a wide range of responsibilities that include but are not limited to:
1. Conducting initial assessments of new patients and developing individualized care plans.
2. Providing direct patient care and administering medications as prescribed by the physician.
3. Monitoring patient's condition, notifying physicians of changes, and adjusting care plans as needed.
4. Educating patients and their families on disease management, self-care, and home safety measures.
5. Coordinating with interdisciplinary team members to ensure comprehensive care.
6. Maintaining accurate, detailed reports and records and complying with healthcare regulations.
7. Demonstrating a commitment to patient safety and privacy by adhering to all regulatory guidelines.
Qualifications:
To be considered for this critical role, you must meet the following qualifications:
1. Must hold a current and unrestricted RN license.
2. Previous RN experience, ideally in Home Health, but not required
3. Exceptional clinical skills with the ability to work independently and make critical decisions.
4. Excellent interpersonal and communication skills, with the ability to build relationships with patients and their families.
5. Strong organizational and time-management skills, with the ability to manage multiple patients simultaneously.
6. Must have a reliable means of transportation as this position requires traveling to patients' homes.
7. Basic computer skills for documentation and electronic medical record keeping.
8. A deep commitment to providing compassionate, ethical, and comprehensive care to all patients.
