What You’ll Be Doing
As a Home Health RN, you’ll provide skilled nursing care to patients in the home setting while helping coordinate care and improve outcomes.
Responsibilities include:
- Performing patient assessments and ongoing nursing evaluations
- Developing and updating individualized care plans
- Administering treatments and medications as needed
- Educating patients and families on disease management and care needs
- Coordinating with physicians and the interdisciplinary care team
- Maintaining timely and accurate clinical documentation
- Helping patients remain safely and comfortably in their home environment
- Active RN license
- Reliable transportation required
- Strong clinical skills and compassionate patient approach
- Ability to work independently and manage your day effectively
- Excellent communication and documentation skills
- Home health experience is a plus, but the company’s training is a major advantage
