Job Details
- Evaluate and assess patients with injuries, illnesses, or post-surgical conditions to determine functional limitations and rehabilitation needs
- Develop individualized treatment plans focused on improving mobility, strength, balance, and functional independence
- Provide hands-on therapeutic interventions including gait training, mobility training, therapeutic exercise, and manual therapy
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and case managers to coordinate patient care
- Educate patients and families on recovery goals, home exercise programs, and injury prevention strategies
- Monitor patient progress and adjust treatment plans based on clinical outcomes and functional improvements
- Document evaluations, treatment plans, and patient progress accurately within the electronic medical record (EMR)
- Assist with discharge planning, including recommendations for home health, outpatient therapy, or rehabilitation facilities
- Ensure compliance with hospital policies, safety standards, and regulatory requirements
- Maintain patient safety during transfers, ambulation, and therapeutic activities
