The Opportunity
We are seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) for our Adult Medicine primary care practice in Hartford, CT. This role is perfect for a clinician who thrives in a community-based setting and values long-term patient relationships.
Key Responsibilities
Clinical Excellence: Perform comprehensive physical exams and assessments on a diverse patient population.
Patient Management: Diagnose and treat acute and chronic illnesses and injuries with an evidence-based approach.
Diagnostics: Order and interpret diagnostic tests and imaging studies to inform care plans.
Pharmacology: Prescribe medications and treatments as necessary, maintaining high safety standards.
Education: Empower patients through counseling on health promotion and disease prevention.
Collaboration: Partner with a multi-disciplinary team to develop and implement holistic treatment plans.
Documentation: Maintain accurate, real-time medical records to ensure continuity of care.
What You Bring
Licensure: Currently licensed (or eligible) as a Nurse Practitioner or Physician Assistant in the State of Connecticut.
Experience: Previous practice experience in Primary Care, Family Medicine, or Internal Medicine is preferred.
Soft Skills: Excellent interpersonal skills and the ability to handle a high-volume outpatient setting while maintaining a patient-centered focus.
Certification: Board Certification is highly desired.
