Based in our family of primary care clinics in the Pomona Valley region, you'll play a key role in delivering high-quality, compassionate care to adult patients across a broad range of needs. You'll work alongside a collaborative team of clinicians, benefit from integrated virtual support, and be empowered to make a real impact on the health of our communities.

You'll work alongside a collaborative team of clinicians, benefit from integrated virtual support, and be empowered to make a real impact on the health of our communities.
What You'll Do
- Manage a patient panel, delivering care across acute, chronic, and preventive visits
- Provide in-person care at a clinic site with occasional telehealth appointments as needed
- Collaborate with in-office teammates during daily huddles and virtual clinical team check-ins
- Perform in-office procedures aligned with your clinical training and credentials
- Support patients in their care journey by coordinating referrals and follow-ups with specialists and facilities
- Maintain accurate and timely patient documentation within our electronic medical records system
- Contribute to a team culture of quality, collaboration, and continuous improvement
- Completed an accredited NP or PA program with current national certification (required)
- Licensed to practice in the state of California (required)
- In the past 5 years, you have:
- 2 years of experience as an Advanced Practitioner in an outpatient primary care setting, OR
- 1 year in outpatient primary care and either 1 year in a primary care fellowship or 1 year in urgent care (required)
- Strong diagnostic and clinical decision-making skills
- Excellent communication, patient education, and interpersonal abilities
- Comfortable working in a team-based environment and adapting to a fast-paced clinic setting
Salary range
$130,000 - $150,000 USD
